Licensed Social Worker or RN - Psychiatric Hospital Coordination

Location: Los Angeles, CA (Hybrid)

Schedule: Monday-Friday, 8:00 AM-5:00 PM

Hours: 30 Hours Per Week, No Weekends

Compensation: $50-$55 per hour


Key Responsibilities

  • Consult with individuals receiving services, caregivers, and support teams regarding mental health concerns, psychiatric hospitalizations, and treatment needs.
  • Communicate regularly with hospital personnel regarding admission diagnoses, treatment progress, discharge planning, and follow-up care.
  • Conduct hospital visits as needed to support care coordination and continuity of services.
  • Participate in discharge planning meetings and assist with developing appropriate post-discharge support plans.
  • Collaborate with psychiatrists, psychologists, behavioral health professionals, case managers, and interdisciplinary teams regarding treatment recommendations and ongoing care needs.
  • Assist individuals and families in identifying and accessing outpatient mental health services following hospitalization.
  • Coordinate follow-up care and community supports to promote successful transitions and reduce barriers to treatment.
  • Maintain accurate and timely documentation within electronic records systems and prepare required reports.
  • Serve as a liaison between hospitals, mental health agencies, caregivers, providers, and support teams.
  • Participate in team meetings and provide updates regarding active cases and discharge planning efforts.
  • Support continuity of care and advocate for appropriate behavioral health services and resources.
  • Perform other related duties and special projects as assigned.


Required Qualifications

  • Master's Degree in Psychology, Social Work, Marriage and Family Therapy, or a related behavioral health field.
  • Active California licensure as a Social Worker (LCSW/ASW as applicable) or Registered Nurse (RN).
  • Minimum of two (2) years of experience working as a licensed social worker or behavioral health professional.
  • Strong understanding of psychiatric treatment, discharge planning, and community mental health services.
  • Excellent communication, care coordination, and relationship-building skills.
  • Ability to work effectively with individuals, families, healthcare providers, and interdisciplinary teams.
  • Strong organizational skills with the ability to manage multiple cases and priorities simultaneously.
  • Proficiency with electronic documentation systems and Microsoft Office applications.


Preferred Qualifications

  • Experience working in psychiatric hospitals, inpatient behavioral health, crisis services, or community mental health settings.
  • Experience supporting individuals with intellectual and developmental disabilities (IDD).
  • Familiarity with mental health agencies, behavioral health systems, and community-based resources.
  • Experience collaborating with psychiatrists, therapists, hospitals, and behavioral health providers.
  • Knowledge of care transitions, discharge planning, and behavioral health case management.


Work Environment & Schedule

  • Hybrid work environment with a combination of remote work and community-based responsibilities.
  • Monday-Friday schedule with 30 hours per week.
  • No weekends or on-call responsibilities.
  • Travel to hospitals, provider locations, and community settings may be required.
  • Collaborative interdisciplinary team environment with ongoing clinical support and consultation.
