The Columbus Organization supports individuals with complex care needs through person-centered services that promote independence, community inclusion, and improved quality of life.
The Support Coordinator assists individuals with intellectual and developmental disabilities in accessing, coordinating, and monitoring community-based services. This role focuses on care coordination, service planning, and ongoing support to help individuals achieve personal goals and maintain quality outcomes.
· Coordinate and manage individualized service plans (ISP)
· Conduct assessments and gather clinical/social information
· Facilitate ISP meetings and ensure annual updates
· Connect individuals to community resources and services
· Assist with Medicaid eligibility and recertification
· Maintain monthly face-to-face contact per guidelines
· Monitor service delivery and outcomes
· Document services and maintain compliance with DBHDD and HIPAA
· Respond to participant needs, concerns, and crises
· Collaborate with families, providers, and interdisciplinary teams
· Bachelor’s degree in human services field (required)
· 1+ year experience in case management or developmental disabilities services
· Master’s degree may substitute for experience
· Valid driver’s license and ability to travel locally
· Must reside within assigned coverage area (preferred)
· Care coordination and case management
· Strong communication and relationship-building
· Problem-solving and critical thinking
· Knowledge of Medicaid/waiver programs
· Microsoft Office and documentation skills
This is a mobile, field-based position requiring travel throughout the assigned counties. Work includes home visits, community settings, and occasional remote/administrative tasks. Flexibility for evenings or weekends may be required.
